The Characteristics of Mild Headache in Migraineurs: Is it Tension Type Headache or Mild form Migraine?

Abstract

|
Background: Migraine and tension type headache are most common type primary headache. But clinical features can
overlap and mild intensity headache of migraineurs can show features of tension type headache. The objective of
this study was to investigate the characteristics of mild headache in migraineurs.
Methods: 32 migraineurs were recruited and completed the headache diary when their headache occurred. Total 292
attacks were divided into three phenotypes; Migraine (MH), Tension type headache (TH) and undifferentiated headache
(UH). In another aspect, all headache attacks were divided into mild and non-mild headache attacks according to
three points of view; severity, disability and rescue regimen usage. We compared the difference in incidence of MH,
TH, UH between mild and non-mild headache attacks.
Results: The overall incidence of MH, TH, and UH were 72.3, 12, and 15.8%. In three aspects of mild headache, most
frequent headache phenotype was MH which occupied 62.5-68.3%. The incidence of headache phenotype was not significantly different between mild and non-mild headache attacks according to headache severity and rescue regimen usage. The mild headache attacks based on headache related disability revealed the different incidence of MH, TH, UH compared to non-mild headache group, which showed the tendency of higher incidence in MH in non-mild headache.
Conclusion: Most frequent clinical phenotype of mild headache in migraineurs is migraine headache phenotype. The result of present study suggests that it is worthy to pay more attention to mild headache in migraine patients for proper management.
